The benefits of a happy, healthy smile are immeasurable! Our goal at Wheatland Family Dental is to help you reach and maintain maximum oral health. Please fill out our patient forms completely and bring them to your first appointment. The better we communicate, the better we can care for you.
We know there are times and situations when you cannot keep your appointment. If this happens, please realize that we need 48 hours notice so we may offer this time for another patient.
At your first appointment, please bring us your insurance information, forms, policy holder’s card, and/or group number to expedite insurance processing. Helping you submit your insurance claim is a courtesy we offer all of our patients.

We accept CareCredit, a dental and health care financing company that helps make dental costs more affordable, with low interest rates and monthly payments. Please ask us about applying for CareCredit and look for brochures in our office.
